Attention All Athletes

Physical and Clearance 

(Must complete all steps to be eligible to Begin Practice) 

STEP 1 (pick one option)

• (Option 1) At home print the new physical form ( must be printed on one page, front and back) will not accept two pages 2021 LAUSD Physical Form.pdf (PDF)

• (Option 2) Come to the front of school and pick up new physical form ( Follow safely precautions) 

Step 2 (both front and back must be completed)

• (Front page) Fill out out front of the physical form, must have parent signature
• (Back page) Doctors office will fill out ( MUST HAVE DOCTORS OFFICE STAMP @ THE BOTTOM) will not accept without STAMP.

Step 3 

• Take a picture of your physical form (front and back) for your records.
• Return the physical Form back to the Main Office ( Put it in the Athletic Directors Mail Box)

Step 4 

• Complete your Online Athletic Clearance  (Link)

  • Fill in all boxes
  • Upload a picture of your insurance card
  • Have parent/guardian sign confirmation sheet.
